connecting FAMILIES Connecting Families is a Networking Support Group that provides opportunities for parents of children with special developmental, physical or mental health needs to gather for support. Activities & support opportunities include: Sensory Friendly Movies Educational Workshops Monthly Support Group Meetings The groups are led by caring facilitators with lived experience and groups are made up of parents and other voluntary members Referrals can be made by: Parents Caregivers Social Workers School Staff Mental Health Providers and Professionals For more information contact PACT for Families at pactoffice@pactforfamilies.org

A community-led prevention system supporting youth and families across our region. Communities That Care (CTC) Program What is Communities That Care (CTC)? Communities That Care (CTC) is a prevention framework that helps communities work together to support youth, strengthen families, and reduce risk factors that lead to substance use, violence, and mental health challenges. CTC gives local people — parents, youth, schools, organizations, and caring adults — a shared roadmap for creating safer, healthier environments where young people can thrive. Our Local CTC Approach CTC in our region is built on collaboration, data, and community voice. We bring together partners who care about youth wellbeing and give them the tools to create meaningful, lasting change. We focus on: Building protective factors that help youth succeed Programs & Initiatives Here are some of the ways CTC supports youth and families in our community: Youth Leadership & Voice We create opportunities for young people to lead, speak up, and shape the solutions that affect them. Youth help guide campaigns, events, and prevention strategies. Parent & Caregiver Support Workshops, resources, and community events that strengthen family connection and communication. Positive Community Norms Campaigns Messaging that highlights the strengths of our youth and community — shifting the narrative toward hope, resilience, and belonging. Community Events & Engagement Seasonal events, outreach activities, and collaborative projects that bring families, partners, and youth together. School & Community Partnerships Working with schools, youth programs, and local organizations to support prevention efforts and build consistent, supportive environments.

COSI Cultural Outreach Support & Initiative The Cultural Outreach & Support Initiative (COSI) provides culturally responsive support for youth (ages 11–24) and their families in Kandiyohi, McLeod, Meeker, Renville, and Yellow Medicine counties. Through community liaisons, trauma-informed education, and interpreter services, COSI addresses mental health and substance use challenges while reducing barriers to care. By fostering trust and engagement, the program uplifts diverse cultural groups and encourages careers in mental health and substance use fields to build a more inclusive future. Who can refer? Referrals to the COSI program can be made by anyone aware of Ethnic and Cultural Minority Youth or young adults affected by chemical dependency or mental health problems. Common Referral Sources Include: School Counselors County Case Managers Corrections agents Treatment Agencies Parents or other family members Friends Youth self-referral

committees The listing below provides links to information on our collaborative committees We collaborate with our partners and communities to help deliver our mission The collaborative is a unique organization developed by its many partners to fill gaps in services and reduce duplication. Members include county social service, public health, and corrections agencies; school districts; mental health providers; and other child-serving agencies. PACT for Families and its partners are dedicated to changing systems to be more prevention-focused and family-friendly. Full Collaborative LEARN MORE early childhood committee The focus of the Early Childhood Committee is to enhance the cognitive, physical and social/emotional development of children from prenatal through age five. The committee looks for ways to help all early childhood agencies and programs work together to meet the needs of our youngest children and families. We strive to identify and address gaps in services. The committee's mission statement is: to enhance early childhood mental health services in the five counties. LEARN MORE The mission of the 5 County Children’s LAC is: to use the knowledge of a broad range of people to improve local mental health services, especially those who use mental health services. Participation is encouraged by family members, those who provide services and those who are concerned about public policy. 5-county children's lac LEARN MORE 9-25 committee The 9 to 25 committee uses data from the SHARE and other surveys/research to identify concerns related to children and adolescents ages 9 to 25 in the PACT five-county area. The 9 to 25 committee divides into topical work groups to develop specific initiatives around the goals of the larger group. While mainly dealing with issues related to children and youth, this group’s focus is on the identified areas of concern rather than any specific age-related group. Their focus is being a voice for children with mental health issues, identifying gaps in services, and networking with parents and organizations in the five counties of PACT for Families and statewide... - LOCAL COLLABORATIVE TIME STUDY | PACT for Families
FUNDING REQUEST GRANT APPLICATION ACTIVITY TRAINING PACT for Families uses Local Collaborative Time Study (LCTS) funding to provide grants to partner agencies. Funding priorities for LCTS grants have been for activities directed at a clearly defined at-risk population such as but not limited to the following; At risk of EBD or SED/ED Economically disadvantaged Abused/neglected At risk of school failure/dropping out Delinquent Chemical use/abuse Mental illness/chemical dependency (MI/CD) Teen mothers At risk of out-of-home placement The focus of an LCTS project must be to reduce the risk of out of home placements. PACT for Families works creatively to provide services and opportunities for youth who are struggling at home, school, or community, and to find resources and outlets to help meet their needs. We know that youth do much better when they receive local support from their community.
