aces (Adverse Childhood Experiences) More Resilient Minnesota Project Community Conversations | Partnership with FamilyWise More Resilient Minnesota seeks to improve the health and resilience of current and future generations. Resilience includes individuals’ inherent strengths and nurtured capacities as well as the resources and supports of their families, communities, and cultures. Following the Self-Healing Communities approach, Children’s Mental Health and Family Services Collaboratives educate their communities about neurobiology, epigenetics, ACEs, and resilience research (NEAR Science) and train local presenters to share this information widely throughout their communities. Collaboratives then move from awareness of Adverse Childhood Experiences (ACEs) to action. They engage and gather input from a diverse representation of community members in their areas through Community Resilience Conversations with groups of people and/or 100 Cups of Coffee Interviews with individuals. This community wisdom and local data guide Collaboratives as they develop Community Resilience Plans. Collaboratives will then have the opportunity to pilot Community Resilience Initiatives outlined in their Community Resilience Plans and also use those plans to leverage funding and provide strategic direction for their activities. road to success Operating within Kandiyohi, McLeod, Meeker, Renville, and Yellow Medicine counties, RTS (Road to Success is a collaboration between PACT for Families Collaborative and member organizations, focusing on enhancing youth resilience and support systems. Our dedicated team offers personalized assistance through non clinical one-on-one sessions facilitated by Transition Specialists, Peer Support, and Family Liaisons. Using the evidence-based Life Course Model, our staff aims to foster greater self-sufficiency and independence to individuals aged 16-25 facing recognized SED/SMI, emerging mental health issues, or experiencing developmental disabilities. This comprehensive approach has consistently improved real life outcomes, enabling individuals to achieve their goals and successfully transition into adult roles and responsibilities. Clients in our Transition Services Programs receive support in the following areas: Road to Success is a federally funded program under SAMHSA (Substance Abuse and Mental Health Services Administration) aimed at providing transition services for youth ages 16-25. These programs specifically cater to individuals who are at risk of Serious Emotional Disturbances (SED), Serious Mental Illness (SMI), or experiencing developmental disabilities. The CEO Board meets at least annually before the annual meeting of the Collaborative and at other times as necessary. mission: The Board of Chief Elected Officials gives authority to either the PACT for Families Director or the Executive Board Chair to enter into any contract or to execute any instrument for the agency. - LOCAL COLLABORATIVE TIME STUDY | PACT for Families
FUNDING REQUEST GRANT APPLICATION ACTIVITY TRAINING PACT for Families uses Local Collaborative Time Study (LCTS) funding to provide grants to partner agencies. Funding priorities for LCTS grants have been for activities directed at a clearly defined at-risk population such as but not limited to the following; At risk of EBD or SED/ED Economically disadvantaged Abused/neglected At risk of school failure/dropping out Delinquent Chemical use/abuse Mental illness/chemical dependency (MI/CD) Teen mothers At risk of out-of-home placement The focus of an LCTS project must be to reduce the risk of out of home placements. PACT for Families works creatively to provide services and opportunities for youth who are struggling at home, school, or community, and to find resources and outlets to help meet their needs. We know that youth do much better when they receive local support from their community.
