    PROGRAMS The listing below provides links to current major projects of PACT for Families Collaborative. Many PACT for Families programs are funded by grants from state and federal agencies. Others are paid for through Local Collaborative Time Study (LCTS) dollars and membership dues. Although the Collaborative strives to retain all programs that are showing positive benefit to youth and families, some programs do end when the funding source ends. PACT for Families provides a large network to share ideas and resources and to seek funding for new and continuing local efforts. By being involved with collaborative committees and connections, people are automatically connected to new ideas and resources.   • Road to Success | PACT for Families

    road to success Operating within Kandiyohi, McLeod, Meeker, Renville, and Yellow Medicine counties, RTS (Road to Success is a collaboration between PACT for Families Collaborative and member organizations, focusing on enhancing youth resilience and support systems. Our dedicated team offers personalized assistance through non clinical one-on-one sessions facilitated by Transition Specialists, Peer Support, and Family Liaisons. Using the evidence-based Life Course Model, our staff aims to foster greater self-sufficiency and independence to individuals aged 16-25 facing recognized SED/SMI, emerging mental health issues, or experiencing developmental disabilities. This comprehensive approach has consistently improved real life outcomes, enabling individuals to achieve their goals and successfully transition into adult roles and responsibilities. Clients in our Transition Services Programs receive support in the following areas: Road to Success is a federally funded program under SAMHSA (Substance Abuse and Mental Health Services Administration) aimed at providing transition services for youth ages 16-25. These programs specifically cater to individuals who are at risk of Serious Emotional Disturbances (SED), Serious Mental Illness (SMI), or experiencing developmental disabilities.     MN DHS collaborative Collaboratives There are currently 89 Children’s Mental Health and Family Services Collaboratives in Minnesota: 8 Children’s Mental Health Collaboratives 42 Family Services Collaboratives 39 Integrated Children’s Mental Health and Family Services Collaboratives Collaboratives were envisioned and created as an approach to: Address the needs of children and youth faced with complex problems Maximize results and resources by enhancing coordination and reducing duplication among systems Involve communities, particularly families, in system redesign and implementation so their needs are effectively met Collaboratives Map This is a map that outlines the children's mental health and family services collaboratives VIEW MAP Children's Mental Health In order to better meet the needs of children with emotional or behavioral disturbances or who are at risk of such disturbances, it is the intent of the legislature to establish an integrated mental health service system.   • 5-County Children's LAC | PACT for Families

    5-COUNTY CHILDREN'S LAC What is a Children's Local Advisory Council? Mission: The purpose of the LAC is to use the knowledge of a broad range of people to improve local mental health services, especially those who use mental health services. As well, participation is encouraged by their family members, those who provide services and those who are concerned about public policy. Minnesota statutes require counties to establish local mental health advisory councils or mental health subcommittees of other councils. The Act gives LACs a broad role in the review, evaluation and planning of local systems. Duties: • Recommend: Make recommendations regarding services provided by the local children’s mental health system. The LAC is to provide the county board with a report of unmet mental health needs of children in the county at least annually. • Plan: The LAC is to be involved in the development of mental health planning for the counties. • Coordinate: Receive input from providers regarding coordination of care between services. • Inform: Identify for the County Board individuals and agencies to receive information on predictors and symptoms of emotional disturbances, where mental health services are available in the county, and how to access the services. • Identify Needs: Seek input from parents, former consumers, providers and others about the needs of children with emotional disturbance in the local area and services needed by families of these children. • Report: The LAC is to report its recommendations regarding the local mental health system to the State Advisory Council on Mental Health. • Other o Policy Recommendations o Develop and conduct public educations activities on behalf of the county board.

The School Network is a monthly virtual gathering designed to help districts stay informed, aligned, and supported in their mental health and substance use prevention work. What the School Network Provides Clear guidance on state mental health and prevention requirements Short, practical sessions (30–45 minutes) that fit into busy schedules Evidence‑based strategies and tools schools can use right away Templates, frameworks, and resources for planning and implementation Opportunities for shared learning across districts Who Participates School administrators, counselors, social workers, prevention staff, public health partners, and anyone supporting school‑based mental health and prevention.
