The mission of Reentry Ready is to support successful transition back to  home communities for offenders from the ages of 16 to 23 through effective case management and job readiness.

The focus is to involve participants who are medium to high risk for re-offending without significant changes taking place. 

Reentry Ready will utilize a staff position (navigator) to team with correctional placements, probation, mental health agencies, social services, job services and other community resources to develop a comprehensive plan that meets the needs of the offender to transition successfully into the community. The plan will include an evidence-based assessment and will help develop both formal and informal supports to contribute to a successful plan.

The intent is to initially meet with the offender several months before he/she is released from placement to begin identifying what needs to be done to be successful after release.
